CSTB: The Inhibitor Deficient in Unverricht-Lundborg Disease

Cystatin B (CSTB) is a small, ubiquitously expressed, cytosolic cysteine protease inhibitor. CSTB primarily protects cells against unwanted degradation by inhibiting the activity of lysosomal cysteine cathepsins (Cathepsin B, L, and S), which can leak into the cytoplasm. CSTB is predominantly localized to the cytoplasm and nucleus and is expressed highest in the brain. Loss-of-function mutations in the CSTB gene result in Unverricht-Lundborg disease (EPM1), a progressive form of myoclonus epilepsy. Disease pathology has been attributed to the unregulated activities of Cathepsins, particularly in neurons, which may lead to increased apoptosis and inflammation.
