CYP2C19: A Key Polymorphic Drug Metabolizer
Cytochrome P450 2C19 (CYP2C19) is a monooxygenase enzyme present in the endoplasmic reticulum of liver cells. This enzyme has an extremely high genetic polymorphism, which segregates the population into poor, intermediate, extensive, and ultrarapid metabolizers. It is a major human drug metabolizer and is responsible for processing about 15% of the drugs in clinical use. These include drugs like proton pump inhibitors (PPIs) and most importantly the anti-platelet pro-drug clopidogrel (Plavix). Non-active form of clopidogrel needs to be converted into its active form by CYP2C19 enzyme. Poor metabolizers of clopidogrel (carrying gene variants) are unable to activate the drug properly, which increases their risk of heart attack and stroke manyfold. This is why CYP2C19 is a textbook example of a pharmacogenomic target.

Background
This gene encodes a member of the cytochrome P450 superfamily of enzymes. The cytochrome P450 proteins are monooxygenases which catalyze many reactions involved in drug metabolism and synthesis of cholesterol, steroids and other lipids. This protein localizes to the endoplasmic reticulum and is known to metabolize many xenobiotics, including the anticonvulsive drug mephenytoin, omeprazole, diazepam and some barbiturates. Polymorphism within this gene is associated with variable ability to metabolize mephenytoin, known as the poor metabolizer and extensive metabolizer phenotypes. The gene is located within a cluster of cytochrome P450 genes on chromosome 10q24.
