HMGCR: A Central Enzyme in Cholesterol Metabolism and Cardiovascular Disease
HMGCR serves as a pivotal regulator in human pathophysiology through its rate-limiting role in cholesterol biosynthesis. This membrane-bound enzyme's dysregulation underlies various metabolic disorders, particularly hypercholesterolemia and atherosclerosis, where excessive activity contributes to plaque formation and cardiovascular complications. As the primary target of statin drugs, its inhibition represents a cornerstone in cardiovascular disease prevention, though genetic variations can influence therapeutic responses. Beyond lipid metabolism, aberrant expression patterns correlate with several malignancies, including breast and prostate cancers, where upregulated activity supports tumor growth through both cholesterol-dependent and independent mechanisms. The enzyme's complex regulation, involving sterol-sensing domains and post-translational modifications, creates multiple pathological vulnerabilities - mutations affecting these regulatory elements lead to rare inherited lipid disorders. Recent research reveals connections to neurological conditions, with altered activity observed in Alzheimer's disease and other neurodegenerative processes, potentially through impacts on membrane integrity and synaptic function. Background
HMG-CoA reductase is the rate-limiting enzyme for cholesterol synthesis and is regulated via a negative feedback mechanism mediated by sterols and non-sterol metabolites derived from mevalonate, the product of the reaction catalyzed by reductase. Normally in mammalian cells this enzyme is suppressed by cholesterol derived from the internalization and degradation of low density lipoprotein (LDL) via the LDL receptor. Competitive inhibitors of the reductase induce the expression of LDL receptors in the liver, which in turn increases the catabolism of plasma LDL and lowers the plasma concentration of cholesterol, an important determinant of atherosclerosis. Alternatively spliced transcript variants encoding different isoforms have been found for this gene.
