NR1H4: The Master Regulator of Bile Acid Metabolism
Nuclear Receptor Subfamily 1 Group H Member 4 (NR1H4), also known by the universal name Farnesoid X Receptor (FXR), is a nuclear receptor that acts as the body's sentinel for bile acids. Bile acid-FXR complex dimerizes with RXR in the liver and intestine and binds to DNA to transcriptionally control an extensive network of genes that govern every aspect of bile acid synthesis, transport, and detoxification. In addition to controlling bile acid homeostasis, FXR activation reduces triglycerides, enhances insulin sensitivity, and exerts anti-inflammatory and anti-fibrotic effects in the liver. Because of this, it has become a prime therapeutic target for cholestatic liver diseases and non-alcoholic steatohepatitis (NASH).

Background
This gene encodes a ligand-activated transcription factor that shares structural features in common with nuclear hormone receptor family members. This protein functions as a receptor for bile acids, and when bound to bile acids, binds to DNA and regulates the expression of genes involved in bile acid synthesis and transport. Alternatively spliced transcript variants encoding different isoforms have been described.
