HAO1: A Key Target in Oxalate Metabolism and Rare Kidney Disorders
HAO1 is a peroxisomal enzyme primarily involved in the oxidation of glycolate to glyoxylate, playing a key role in glyoxylate and oxalate metabolism. Dysregulation of HAO1 has been closely linked to primary hyperoxaluria type 1 (PH1), a rare genetic disorder characterized by the overproduction of oxalate, leading to kidney stones, nephrocalcinosis, and eventually renal failure. In PH1, inhibition of HAO1 has emerged as a promising therapeutic strategy to reduce hepatic oxalate production and prevent disease progression. Additionally, altered HAO1 expression or activity may have implications in other metabolic disorders related to peroxisomal dysfunction. Recent studies have also explored HAO1 as a target for RNA interference therapies, aiming to restore metabolic balance in affected individuals. Given its central role in oxalate synthesis and peroxisomal metabolism, HAO1 is gaining attention as both a biomarker and therapeutic target in metabolic kidney diseases and rare genetic conditions.

Background
This gene is one of three related genes that have 2-hydroxyacid oxidase activity yet differ in encoded protein amino acid sequence, tissue expression and substrate preference. Subcellular location of the encoded protein is the peroxisome. Specifically, this gene is expressed primarily in liver and pancreas and the encoded protein is most active on glycolate, a two-carbon substrate. The protein is also active on 2-hydroxy fatty acids. The transcript detected at high levels in pancreas may represent an alternatively spliced form or the use of a multiple near-consensus upstream polyadenylation site.
