Butyrylcholinesterase (BCHE) is a major plasma enzyme in humans with a well-known function of hydrolyzing choline esters, and also metabolizing a number of clinically important drugs. BCHE: The "Pseudo" Cholinesterase of Human Plasma

BCHE, also known as pseudocholinesterase, is a serine hydrolase that is highly expressed in the liver and secreted into the blood where it is present in a concentration ten-fold higher than acetylcholinesterase (AChE). Although it is related in structure to AChE, the enzyme that rapidly degrades acetylcholine at neuronal synapses, BCHE is not found in the nervous system and is not primarily involved in cholinergic neurotransmission. It is known to hydrolyze acetylcholine, but its predominant physiological function is to scavenge and hydrolyze other choline esters and xenobiotics. Clinically, BCHE is important for two main reasons: 1) it is responsible for the metabolism of the muscle relaxant drug succinylcholine, and genetic variants with low enzyme activity cause prolonged paralysis in patients following surgery; 2) it can bind to, and inactivate highly toxic organophosphate nerve agents such as sarin or VX.

Background

Mutant alleles at the BCHE locus are responsible for suxamethonium sensitivity. Homozygous persons sustain prolonged apnea after administration of the muscle relaxant suxamethonium in connection with surgical anesthesia. The activity of pseudocholinesterase in the serum is low and its substrate behavior is atypical. In the absence of the relaxant, the homozygote is at no known disadvantage.
