CAST: The Endogenous Calpain Inhibitor
Calpastatin (CAST) is an intracellular protein that specifically and reversibly inhibits the conventional calpains, calpain-1 and calpain-2. Calpastatin is the only known endogenous inhibitor of these enzymes and does not inhibit other proteases. Calpastatin has multiple inhibitory domains and so a single molecule can simultaneously inhibit more than one calpain molecule. The relative abundance of calpain to calpastatin within the cell is a major determinant of the net calpain activity. In many disease processes such as ischemic injury, this ratio is disrupted (usually via degradation of the inhibitor itself) resulting in unopposed calpain activation and cell death.

Background
The protein encoded by this gene is an endogenous inhibitor of calpains (calcium-dependent cysteine proteases). It has an N-terminal domain L and four repetitive calpain-inhibition domains (domains 1-4) and mediates the proteolysis of amyloid precursor protein. The calpain/calpastatin system has been shown to be involved in a wide range of membrane fusion events from neural vesicle exocytosis to platelet and red-cell aggregation. The protein product is also thought to regulate the levels of expression of genes encoding structural or regulatory proteins. Alternatively spliced transcript variants encoding different isoforms have been described.
