NGF (nerve growth factor) is a neurotrophic factor and a neuropeptide that is primarily involved in the regulation of the growth, maintenance, proliferation, and survival of certain target neurons. In fact, NGF is crucial for the survival and maintenance of sympathetic and sensory neurons because they can undergo apoptosis in the absence of them. However, recent studies have shown that NGF participates in pathways other than those that regulate the life cycle of neurons. When expressed, NGF was initially in the 7S, 130-kDa complex of 3 proteins - [alpha]-NGF, [beta]-NGF and Gamma-NGF (2:1:2 ratio). This form of NGF is also known as proNGF (NGF precursor). The gamma subunit of this complex acts as a serine protease and cleaves the N-terminus of the beta subunit, thereby activating the protein as a functional NGF. NGF binds to at least two types of receptors: tropomyosine receptor kinase A (TrkA) and low affinity NGF receptor (LNGFR/p75NTR). Both are related to neurodegenerative diseases. When NGF binds to the TrkA receptor, it drives homodimerization of the receptor, resulting in autophosphorylation of the tyrosine kinase fragment. This leads to the activation of PI3 kinase, ras and PLC signaling pathways. Alternatively, the p75NTR receptor can form a heterodimer with TrkA, which has a higher affinity and specificity for NGF. Studies have shown that NGF circulates throughout the body through plasma and is important for overall maintenance of homeostasis. NGF seems to promote myelin repair. Therefore, NGF can be used to treat multiple sclerosis. NGF may also be involved in various mental illnesses such as dementia, depression, schizophrenia, autism, Rett syndrome, anorexia nervosa and bulimia nervosa. Fasinumab is a human monoclonal antibody for the treatment of acute sciatica. Phase III clinical trials are currently under way.
