PRKAB1: A Key Metabolic Sensor and Therapeutic Target in Multiple Diseases
PRKAB1, a key regulatory component of AMP-activated protein kinase, functions as an essential metabolic sensor involved in various disease mechanisms. Impairment of this subunit disrupts cellular energy balance, contributing to metabolic disorders such as diabetes and obesity by affecting glucose transport and lipid processing. In oncology, abnormal PRKAB1 signaling supports tumor survival by altering nutrient metabolism under low-oxygen conditions. Cardiovascular diseases, including ischemic heart conditions and atherosclerosis, are worsened by its dysregulated activity, impacting endothelial health and vascular remodeling processes. Neurodegenerative disorders like Parkinson’s disease may result from faulty neuronal energy management, causing oxidative damage and protein aggregation. Moreover, PRKAB1 influences inflammatory pathways, linking it to autoimmune disorders and chronic inflammatory states. While targeting this pathway offers promising therapeutic avenues to reestablish metabolic homeostasis across these diverse illnesses, achieving precise control remains difficult due to its complex, tissue-specific functions and effects.

Background
The protein encoded by this gene is a regulatory subunit of the AMP-activated protein kinase (AMPK). AMPK is a heterotrimer consisting of an alpha catalytic subunit, and non-catalytic beta and gamma subunits. AMPK is an important energy-sensing enzyme that monitors cellular energy status. In response to cellular metabolic stresses, AMPK is activated, and thus phosphorylates and inactivates acetyl-CoA carboxylase (ACC) and beta-hydroxy beta-methylglutaryl-CoA reductase (HMGCR), key enzymes involved in regulating de novo biosynthesis of fatty acid and cholesterol. This subunit may be a positive regulator of AMPK activity. The myristoylation and phosphorylation of this subunit have been shown to affect the enzyme activity and cellular localization of AMPK. This subunit may also serve as an adaptor molecule mediating the association of the AMPK complex.
