Introduction of Exbivirumab

Exbivirumab is a human monoclonal antibody targeting hepatitis B surface antigens (HBsAg) and has been developed for the treatment of hepatitis B infections. Previous clinical trials have demonstrated its efficacy in preventing the re-infection of allografts in liver transplant patients infected with the hepatitis B virus (HBV).

Mechanism of Action of Exbivirumab

Exbivirumab is a human monoclonal antibody that specifically targets the hepatitis B surface antigen. It can form immune complexes with the hepatitis B virus, and Fc receptors present on immune cells have the ability to identify the constant region of IgG antibodies, subsequently leading to the cytotoxic elimination of hepatocytes.

Targeting HBsAg

During acute infection, the immune system produces endogenous antibodies that target three specific antigens associated with the hepatitis B virus (HBV): the HBV core antigen (HBcAg), the HBV e antigen (HBeAg), and the HBV surface antigen (HBsAg). Chronic hepatitis B (CHB) has been linked to the exhaustion of T and B cell responses, with HBsAg identified as a key factor in the immunopathogenesis of CHB.

Antibody-dependent cellular cytotoxicity (ADCC)

Exbivirumab demonstrates the ability to induce ADCC, thereby overcoming the limitations linked to the antigen-specific dysfunction of antibody-mediated protection against HBV infection. Exbivirumab binds with circulating HBs antigen and the remaining free anti-HBs to attach to extracellular HBs antigen on hepatocytes. Fc receptors found on immune cells recognize the constant region of IgG antibodies and eliminate hepatocytes through a cytotoxic process.
